Jeff_cw- Checking the list of CSA physicians from Tennessee , I don't find your John H. Hale. That doesn't mean that he wasn't one, only that the state of Tennessee doesn't know that he was. There were apparently several John Hales serving in the 60th Tennessee Mounted Infantry. I do find a PVT John Hale in Co. F, which seems to be the one on your POW list. The Civil War Soldiers and Sailors System includes him, but does not give a middle initial. None of the other John Hale listings is: "John H.," and there are no listings for a : "Hamilton Hale." It would be interesting to find out if he graduated from some medical school and when. There are other possible explanations, but they are only speculation at this time. The requirements to become a physician were quite different 150 years ago than they are now. It is possible that your John Hale assisted some physician in the field (The state records list a physician associated with the 60th TN. He was: "A.N. Harris.") and became a physician himself after the war. You said that he was a teacher in 1854. I would be interested to know how he is listed in the 1860 Census.
